He most certainly is sane. The character he portrays on TV probably isn't, but that's part of the act to attract and retain viewers, which means cash in his pocket.

I think that the important thing to remember about guys like Bill O'Reilly, Sean Hannity, etc. is that they are first and foremost entertainers and they needs ratings. If acting like a nut job is gonna get him those ratings, he'll act like a nut job. I don't think that Bill O'Reilly is a nutjob (but he plays one on TV).

I have heard, however, that Ann Coulter is completely bonkers.

I think he did a bad job of making a good point. I agree with him that you shouldnt expect protection from those that you tell to get the hell out.
