Two Plus Two Older Archives

I'm "moving up" to $100 6 max tomorrow. I've played a little higher than my normal $50, and have had good results. These are my stats.

I think I'm playing decent, but I'm worried that I have some common leaks that I can't identify. These are all my $20+ losing hands. I don't expect anyone to reply to them all, just point out the glaring mistakes. Thanks!

 Villain was the type of guy to lead the turn again with a pot sized bet with most hands. I thought the min-raise would put me all in on the turn, leaving myself with $4.5 wasn't the plan.
