Default Poker etiquette

This post is stemming from reading about a possible slowroll. It got me to thinking: has anyone really ever agreed on etiquette in poker? I mean, golf seems to have it pretty much universal, but we, as poker players, have endless debates over possible slowrolls, splashes of pots, and the ad nauseum chop/no chop discussion. So...I was wondering if these rules could be established and written down, or...if they already are, where to find them.

Of course, the real trick would be getting people to abide by them, anyway.
