In short it overlays various statistics from your PokerTracker database onto the table. Install the program, start your tables and add them into GT+.

However I would recommend having a look at
PlayerView which is more customisable and doesn't require you to manually add tables (but costs $50), or possibly
Poker Aces Heads Up Display which is free, very similar to PlayerView but still in development.
