QQ loses to KK - unlucky or bad play?

Agree with previous posters. Having said that, given that it appears to be a tight table, you could have tried a cheeky raise pre-flop. Calling is definitely the worst of the three options though.

not raising the flop is the only mistake I can see here, would check river in case someone was intending to check-raise a flush (although with hindsight this might be to overestimate your opponents)

nothing wrong with this. I think the logical interpretation of his 3-bet would be that he had three of a kind, in which case you had sufficient pot odds to draw for the straight or 3 A's and beat him.

I would have called the turn. You had ten outs (4 8's, 4 K's, 2 T's) giving odds of 3.6/1 of hitting, and pot odds of at least 4.75/1 if you called the 3-bet.
