How an additional blind changes the game

At a local, somewhat underground poker room in my area there is a local rule where an additional BB is posted where UTG would normally be. What are some changes in gameplay, if any should be expected?

If the PF raise isn't big enough, or everyone limps in, you can expect 1 more person than normal to come in with horrible-marginal cards and catch a hand that will make you cry.

I think this makes the pre-flop raise even more important when you have a hand.
