If the BB gets knocked out of the tournament, does the player to his left get to be SB next hand and skip being the BB (so the button always moves), or does the game try to preserve everyone getting BB and SB and thus make him the only blind (BB), and the person to his right the button, for one round (then the next round, the button would stay and there would be a SB and BB)?

Or do both approaches exist? If so, which is more common etc?

He (the player to the left of knocked out BB) doesn't get a free ride. He has to post a lone BB (no SB). The normal structure of SB/BB resumes on the next hand.

Most places would have just one big blind. Others (Mohegan used to do it this way) would go 2 big blinds on the next hand, and small-small-big on the hand after that.
