UTG raises. Folded to me on button, I three-bet with JhJs. The two of us see:
4d 6s 5h. He bets, I raise, he three-bets, I call.
Ad. He bets. I call.
5s. He checks. I check.
Should have bet the river, right? What does he have to have?

Your opponent looks like he is playing with KK or QQ. That is my guess anyway. But, I think this is one of those hands where if the opponent is typical and raising from UTG, a check/call from the flop to river would win you the most if ahead and lose you the least if behind.
His 3-bet showed you he was ahead and the A hitting doesn't matter too much in my opinion. I think he already has a made hand, like you, by the flop. But unless he was aggressivly playing 1010, you are beat.
So I wouldn't bet the river when checked to. It seems here fears the ace and surely will not fold to a river bet. A bet will be a bet lost.

good reasoning, but--would you only call a three-bet preflop with KK (and heads up)? (and it doesn't seem like some kind of slow-play not to four-bet because he three-bets the flop, right?)
