the whole EV thing

I am familiar with EV, by reading Sklansky's explanation of it in the tournament for AP - but what I find amazing is that people, especially in the higher stakes forums, seem to be able to determine right off the bat whether a certain play is - or + EV.

How are they making this determination? Or are they guesstemating that based on a speculative call/bet, that in the long haul this might be a loser or a winner?
