I've seen it before at a lower limit... one person with TPWK, and someone on the nut flush draw... or even two flush draws. Cant speak to the limits the question is referring to however.

Ive seen it happen 2 times today at 25NL. Not that exact situation but pretty close. If you dont grind it out down here, youd be shocked at some of the wacky stuff that goes on.
