The three-bet's good. That's going to be a lower two pair or Ax of hearts most of the time. And considering the board and his passivity, he may not cap it with a set of twos or threes, so that makes the decision easier.

River plan is bet/call unimproved regardless of the river card.

Perfect, he most likely has a worse 2 pair.
