If I lend my car out to someone and it breaks down or they get into an accident while they are driving it, do they need to pay up?

Nope. You do. (At least in GA/FL, and I'm assuming elsewhere)

Damn, so I really shouldn't let him use my car. Cuz what if something happens.

Call your insurance company and ask them. I let some friends drive my car back from Las Vegas last time we went there and I just called my insurance company before they left town and they told me that the driver's insurance would cover first, then mine.

As for mechanical repairs, it's your car. If something breaks down, it's likely a long-term problem with your car and not that your friend was abusing it.

from an ethical standpoint

breakdown = no

accident which they caused = yes

This is not my experience at all. Apurva drives Matt's truck. Apurva crashes Matt's truck into Lauren's car, which gets pushed into James's Jeep. Matt's insurance has to cover the entire cost of the accident.

One of my friends was borrowing my truck when my idle air control valve went bad, causing a surging idle. I paid for it.
