Re-rasing all in with mid pairs

Here's a situation typical of what I've faced multiple times in the last few days: blinds are 50/100 and I'm on the button holding 99 with 800 chips. A fairly loose EP player makes it 250 and everyone folds.

What's your standard move/thought process here? This is at the 100s, but I'm curious to hear answers from any level.

4-5 handed, if i'm shortie and there isn't a MONSTER stack in the blinds, i'm gambling.

this example looks a little earlier. 5-6 handed, a few of you in the 8-10 BB range? i'm still patient, waiting for the button to move along a bit, giving me open-push opportunities.

If you have some folding equity against villain, this is a very positive $EV move over time.
