Can someone recommend a quick simple format for taking player notes in tournaments.

For most huge tournaments, though, it's unlikely you'll spend enough time with the same players to get too detailed.
I track players with a shorthand:
Limp = .
Minraise = r
3bb raise = R
Limp, call a raise = ;
From this I can tell at a glance how loose a player is, what their typical raises look like (ie if I see a lot of Rs, and then he min-raises UTG, I know it's time to worry), if he's a total rock, and how likely he is to fold if I try to pwn his limps.
It's a bit of a pain to track every hand for four hours, but I find it keeps me focused.
I also, of course, note cards opponents show down, and how they got to show-down.

I usually do what Sam does, with the exception if I'm playing at party and they give me a high table number. But if I'm at say table #3, this comes in handy
