make it extremely hard to beat HU and 4 player or less situations? What's the net effect over time?

it depends on the limit u are playingand the proportionate size of the rake.

It depends very much on your edge over the weakest other player. Since you'll be playing with the weak player all the time, and rake caps at about 2% of the pot on average in a 10-20 game, you need to have more than 2% advantage.
