explain the blue box (and how it was aquired) and the hideous face (and the guy it frightens).

from what I understand (via movie reviews, talks with others, and multiple, almost maniacle viewings), the blue box represents reality. It only appears in the dream sequences, and is viewed with some sort of trepidation. It's kind of a constant reminder that the ideal life that Diane/Betty has dreamt up is not real. When she Rita opens the box in the dream, it signals reality finally catching up with the dream, and the end of the idealized world.
The disfigured guy and the guy who has a heart attack are just characters that Diane remembers from Winkies when she met the hitman there. She has transferred the evil of her act at winkies into a tangible figure from that sequence, and the guy dying is a manifestation of that evil.

Trying to make sense of or find value in Mulholland Drive is like trying to find edible food in a baby's used diaper.
