Those who read the 4% PFR after 47 hands (AQo) (http://forumserver.twoplustwo.com/showthreaded.php?Cat=&Number=1859870&page=0&view=c ollapsed&sb=5&o=14&fpart=1) post will find the preflop situation very familiar. This hand probably happened after about 40 hands with villain, during which he raised twice (PFR = 5).

What's your flop line?

Check/fold if CO bets again. I don't feel comfortable betting into the turn if a blank hits.

What is his VPIP and AF? I think these stats are just as important as his PFR stats.

If he's passive I might try to get this hand heads up with a check-raise. I check and hope he checks behind on the turn, but if he bets im letting my hand go.

If he's aggresive I check/call the flop and fold the turn U/I.

Sorry, I didn't mean to leave the VPIP out. He was something around 20-30 at this point. His agression numbers would be meaningless after 40 hands.

I disagree, if the villian rarely raises post flop, 40 hands is more than enough to tell that.

5% is certainly not high (could be sample size though). I think that with very few ways to improve I would probably check/fold this flop. We might be in trouble vs AK even if we hit our A, and we won't get odds for the Gutshot, even if a 9, J, or K hits. OOP, I think it's a check/fold. If you were the button here, then I might raise a flop bet to get it HU, and get a free turn.
