Page 185, Dan says "With king-queen I would play in all middle positions, with a mix of 60 percent raises and 40 percent folds." Should it be 40 percent calls?

Could but, but not necessarily.
Doyle Brunson pushes the idea that you either raise or fold. Never just call. This could be what Harrington is talking about. If he's not in a position to raise and play aggressively, he would rather just lay down a hand like KQ.

I think the poster's point was how do you 'play' if you 'fold'. By that logic we all play 100% of hands from any position.

you're right, not really clear, but could mean "i will have the possibility of playing them from all middle positions"

Given that the next sentence says [I'm doing this from memory, as I'm at work, and just read this section a few days ago] something like, "With king-jack, X% raise and Y% calls," I think the 40% "folds" in the sentence you quoted has to be a typo.
