It usually refers to when you hold, say, AQ and the flop comes 4 Q 9. You have the top pair on board, and the top possible kicker. You bet/raise here /images/graemlins/smile.gif.

TPTK would also hold true in this situation.
You Hold A-J... and the flop is A-K-Q.
