Hi,
In a wild hold'em game, where the table is frequently seeing the flop 7 ways for 3 or 4 bets, AND then going off for 3-4 bets again on the flop, Sklansky recommends playing only very big hands (AA-JJ, AK). How does this change if they're ONLY going nuts pre-flop, and then playing reasonably passively on the flop and beyond?

I'd still only play the stronger class of hands simply because there's no point putting a lot of money just to see the flop, even if there are a lot of other fools doing the same. I don't think you can go wrong hanging tight in a situation like that.

I cannot see how small pairs would be unprofitable.

I would think that small pairs would be terrible in this sort of game. With small pairs you want to see the flop really cheaply and then hope lots of money goes in after the flop if you hit your set. If there are tons of bets pre-flop and very few post-flop, that seems to be the opposite of what you want.

I play games like this regularly. I can say for sure that small pairs bad, suited connectors good.
